Family Law

This is a sample motion to waive a home study for a second-parent adoption.

A fill-able .pdf of Maine court form PB-03, the Jurisdictional Affidavit that must be filed in District court with any adoption, minor guardianship, or minor name change case.

The rules require the signature of the Clerk or an attorney. Non-attorneys must get this form from the court. This form is still called CV-035 in places, but is now called CV-030 on the form. Posted for informational purposes only.

  • CV-037 Subpoena for Hearing on Motion for Contempt 6/14

The rules require the signature of the Clerk or an attorney.  The Maine Court website states that a hard copy of this form must be purchased from the court clerk for $5.

Not interactive. The rules require you to use an original paper summons from the clerk's office.

You will also need to prepare this form: CV-191 Supporting Affidavit 6/2020
NOTE: The court rules state that a person whose income is from "poverty-based public assistance programs" is presumed to qualify.  Learn more here.

Must be filed with Notice of Appeal.
