Protection From Abuse Forms

• PA-001 Complaint for Protection From Abuse Includes, as needed, all other necessary forms: PA-005 - Service Information, PA-015 - Affidavit for Confidential Address, FM-017 - Important Notice to Plaintiff, and FM-050 Child Support Affidavit.

Family Law - Answer and Counterclaim

Although the Maine courts do not provide standard Answer forms for defendants in family law cases (and the court will accept FM-020 Entry of Appearance in lieu of an Answer), we have prepared these forms for defendants who wish to file a more formal Answer and Counterclaim:

• Answer and Counterclaim for Divorce (no minor children involved)
