Tech tips for using .pdf interactive forms
Most computers have Adobe Acrobat Reader, which
you can use to view, print or download the blank forms. Or you can fill out
the interactive forms on-line, and print your completed form. Click on
the form you want and it will open in your Reader.

For the more complex forms, like the supplemental child support worksheet, we recommend
using the most recent version.
Want to save your completed form?
When you fill out our forms using Adobe Acrobat Reader, you can print them.
But you won't be able to save them. To save completed forms, we recommend using
Foxit Reader or CutePDF.

Court Instruction Sheets
The forms in this section are "interactive"; you can
fill them out online. The "self-calculating" forms
(child support affidavit, child support worksheet, financial statement) require Adobe Acrobat
Reader version 4.0 for the self-calculating function to work. We want to make
these work well for our staff, volunteer attorneys and pro-se clients. Please
let us know ways we can improve how these interactive forms work. Send
us your ideas.

Important! The Probate Court rules set special forms standards.
- Forms with multiple pages must be double-sided.
- Forms must be printed on 20 lbs. acid-free natural-white (not bright white) 25% cotton fiber paper or better.
- Forms must be printed with 600 dot per inch black laser printer or better.
You will be charged a fee for the forms when filed with the court.
Forms 1 to 4 pages: $1.00 each; Forms more than 4 pages: $2.00 each.
If you do not follow these rules, the court will not accept your papers for filing. You can buy
regulation forms from the Probate Court if you need to.
