These are the forms you will need to start a court case. We post forms for the most common types of family law cases (divorce and parental rights), protection from abuse, protection from harassment, small claims, name change and guardianship of a minor.

The Civil Rules require you to use an original summons from the clerk's office. 

Defendants in more serious criminal matters (where jail term could result) can use this form to apply for a free court-appointed lawyer. 

You will also need to include these forms for the Court

Probate Court Forms
